Actually, it has happened a lot. It's happened in the USA, in the UK, and even in Pakistan! Just because you didn't see a news blurb about it, doesn't mean it didn't happen. Further, I run into a lot of Muslims in my line of work, and to date all of them (every single one) has made a point to denounce Al-Qaeda and suicide bombings as a way to resolve problems.
The point is, you are painting with a really wide brush.
