You bring up a lot of good points.

If it is determined that an illegal has received aid from an emergency room, and it is determined that that illegal has a job, illegally, then make the employer pay for the emergency room visit as a penalty for hiring the illegal in the first place.

If there is a public option for legal immigrants, or private insurance, for that matter, and there is a mandatory requirement that immigrants, along with the rest of us, have insurance, similar to mandatory liability insurance on our cars, then the only ones without insurance would be illegals, and those who are able to get jobs, if they receive aid, must be covered by their employer.

I'm not sure I've said this as well as it could be said, but perhaps someone else could elaborate.
